The goal of Age Management Medicine is healthy aging. The goal of ComiteMD, Age Management specialists, is to place each patient in the lowest possible risk category for diseases such as heart disease, cancer, stroke and diabetes – to optimize your current health and minimize the medical issues that arise with aging. Taking into account your lifestyle and family history, Dr. Comite and her team will develop a personal, multi-faceted health plan that integrates exercise and nutrition, hormonal and metabolic optimization to offer you the best opportunity for a healthy, more vigorous life.

 

Step 1: Getting Started

Once you have made the decision to pursue healthy aging with ComiteMD, you and a ComiteMD team member will coordinate appointments that culminate in an Executive Health Evaluation, your personal, in-depth health appraisal. The Executive Health Evaluation establishes a baseline from which Dr. Comite can determine whether metabolic imbalances exist and, if they do, customize an individualized program to correct them.

 

Step 2: The Executive Health Evaluation

Gathering Information

Information for your Executive Health Evaluation is obtained through:

  • • A comprehensive medical assessment which includes blood, urine, bone density and body composition tests.

  • • Review of your existing medical records.

  • • Completion of a detailed health and lifestyle questionnaire.

Telemedicine Conference Call to Review Information

Following her review of the above information, Dr. Comite will schedule a telemedicine (“telemed”) conference call to interpret your results and offer preliminary recommendations. The telemed call, however, offers you significantly more than an explanation of your lab reports. Dr. Comite will help you connect the “dots” – your medical reports, lifestyle and family background – into a comprehensible whole. In partnership with Dr. Comite, you will determine whether or not you and the program are a good fit. You will also be fully prepared to make the most out of the office visit – your first to ComiteMD offices – which follows the telemed call.

Office Visit

An intensive, four-hour office visit establishes baseline medical functions and parameters. In the office, Dr. Comite and her clinical team conduct assessments which, with the data previously collected, will culminate in a series of final medical recommendations that comprise your personal age management program. Recommendations encompass nutrition, nutritional supplements, exercise, and hormonal and metabolic optimization. The office visit will conclude with a team meeting to review, clarify and answer your questions about the program. The meeting ensures that you and the ComiteMD team are on the same page when it comes to your health goals and next steps. You will leave the office with a comprehensive ComiteMD Health PortfolioTM. Following your office visit, a detailed letter of your evaluation results and recommendations will be sent.

 

Step 3: Follow-up Calls

Shortly after commencing the program, the clinical team will set up a mutually-agreed- upon schedule of follow-up calls to monitor your progress. Each call provides an opportunity for you to review your goals, ask questions and determine whether program adjustments are needed.

 

Step 4: Follow-up Office Visits

Ten-to-twelve weeks after the program begins, an office visit with the Exercise Physiologist is scheduled to assess your metabolic progress. Repeat visits with the Exercise Physiologist will regularly take place throughout the year.

 

Step 5: Continued Monitoring by the ComiteMD Team

Dr. Comite and her team will continue to supervise your progress via scheduled telemedicine calls, bloodwork and office visits as needed during the course of approximately one year. During this time the ComiteMD team will continue to serve as your personal health advisors.

 

Step 6 and Beyond: Sustaining your Gains and Continuing to Age Healthily

Following the approximately one year period described above, the ComiteMD team will schedule a second Executive Health Evaluation. This evaluation serves three critical purposes. It:

1. provides a comprehensive comparison to your baseline Executive Health Evaluation to show the degree of improved health;

2. pinpoints your current state of health and raises a warning bell if potential new aging issues are identified; and

3. like the Executive Health Evaluation before it and any that come after, it plays a key role in determining the steps necessary to stay on the path to healthy aging.
